THE POSITION

The D&A Case Management Specialist provides comprehensive Screening and Assessment, Level of Care Determinations and Resource Coordination for those in need of services. DESCRIPTION OF WORK
This position supports individuals as they seek treatment and related services for substance use disorders. It plays a key role in connecting people to care and monitoring their progress. As a Drug and Alcohol Case Management Specialist, you will perform the following duties:
  • Initial Screening: Conduct an initial review of referred individuals to assess the need for immediate care
  • Eligibility Review: Determine and verify eligibility for public funding and identify needed services
  • Assessment Work: Perform a Level of Care Assessment to understand treatment needs
  • Documentation Tasks: Complete required documentation according to state and county policies
  • Service Linkage: Facilitate connections to treatment providers and related services
  • Progress Monitoring: Maintain contact with individuals and providers to track treatment needs and service delivery
Work Schedule and Additional Information:
  • Full-time employment
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
  • Hours may vary outside of the standard based on the need to be available to referring entities.
  • Telework: You may have the opportunity to work from home (telework) part-time; after successful probationary period completion and regular status is obtained, up to two remote work days may be possible when the demands of the unit are met. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Beaver Falls. The ability to telework is subject to change at any time. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS

Minimum Experience and Training Requirements:
  • One year as a Drug and Alcohol Case Management Specialist Trainee; or
  • A Bachelor's Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or a related field; and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting; or
  • A bachelor's degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice or other related social sciences; and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting; or
  • Any equivalent combination of experience and training.
