As a Community Health Worker, you will serve as a bridge between patients, healthcare providers, and community resources. You'll help individuals navigate complex healthcare systems, overcome social and economic barriers, and gain access to services that improve their overall well-being.

Whether you're helping someone find transportation to a medical appointment, connecting a family with food assistance, educating patients about chronic disease management, or advocating for community needs, you'll play an essential role in improving health outcomes and creating lasting change.

• Build trusting relationships with patients and families
• Connect individuals to healthcare services and community resources
• Help address social drivers of health such as food insecurity, transportation, housing, and access to care
• Provide health education and support in a culturally sensitive manner
• Collaborate with care teams to improve patient outcomes
• Support quality improvement initiatives and patient-centered care efforts
• Advocate for the needs of patients and communities

• High School Diploma or equivalent required
• Bachelor's degree in Human Services or a related field preferred
• Healthcare or human services experience preferred
• Knowledge of community resources or willingness to build community partnerships
• Passion for serving others and improving community health
